Care Essentials Privacy Notice & Consent
Warrior Excel uses a privacy-by-design approach for the Care Essentials Assistance Program. The program is designed to collect only information reasonably necessary to communicate with applicants, review requests, coordinate approved fulfillment, document required training, and administer partnerships.
Information We Intend to Collect
- Basic contact information needed to respond to a request.
- ZIP code or general service area for eligibility, rural-access planning, and distribution.
- Applicant relationship, such as warrior, parent/guardian, or caregiver, when needed for program administration.
- Requested Care Essentials and non-clinical fulfillment information.
- For TENS equipment, only the minimum training/certification status needed to enforce the program safety requirement.
- A shipping address only after approval when shipment is necessary.
Information We Do Not Request Through Care Essentials Intake
Do not submit Social Security numbers, insurance records, medical records, medication lists, detailed pain histories, clinical notes, financial-account credentials, or other unnecessary sensitive information. If information is not needed to administer the program, Warrior Excel’s goal is not to collect it.
Consent & Communications
Program communications and optional marketing communications should be treated separately. Applying for assistance does not by itself authorize promotional email. When intake is activated, applicants will be asked to acknowledge the program privacy notice and separately choose whether they want optional news or marketing communications.
How Information May Be Used
Information may be used to review a request, communicate about the request, coordinate training, match available resources, arrange approved fulfillment, prevent duplicate or fraudulent requests, maintain program records, and create aggregated or de-identified impact reporting. Access should be limited to authorized personnel and service providers who need the information for those purposes.
Your Choices
When the secure intake system is activated, Warrior Excel will provide a method for applicants to ask about program information associated with their request and to request correction or deletion where appropriate and legally permitted.
