TENS Training Acknowledgment
Warrior Excel requires completion of the applicable Warrior Wellness training and certification before a TENS unit supplied through the Care Essentials program is released for program-supported use. This requirement is a safety and program-control measure.
Required Training Gate
A TENS request will be tracked through training statuses such as Certification Required, Training Scheduled, Completed, and Verified. The program workflow is designed so that the TENS item cannot advance to final fulfillment until the required training has been verified.
Participant Acknowledgments
- I understand that Warrior Wellness training does not replace the manufacturer’s instructions.
- I understand that training does not determine whether TENS is medically appropriate for me.
- I will seek guidance from a qualified healthcare professional about whether I should use electrical stimulation and will follow any applicable medical restrictions.
- I will not treat completion of Warrior Wellness training as medical clearance, diagnosis, or treatment.
- I will use program-provided equipment only as instructed and will stop use and seek appropriate help if I experience a concerning reaction or problem.
Minimal Training Record
The Care Essentials system should retain only the operational training information needed for the program, such as training status, completion date, verification status, and authorized verifier. Medical justification or detailed clinical information should not be stored in the training record.
